MySQL databases are created using your cPanel Control Panel.
Usually this is a 3 step process:
- Create the Database.
- Create a Database User.
- Assign Rights for the Database User to the Database.
All 3 of these steps are done from inside cPanel.

NOTE FOR PHPMYADMIN USERS: While you can use phpMyAdmin to manage your MySQL databases, you cannot create databases from within phpMyAdmin -- you must create the database first from within cPanel.
